Home Health

Sometimes you need a helping hand while you are recovering from an illness or managing your condition after hospitalization. Rockcastle Regional's Home Health professionals offer comprehensive, advanced care in your home. Services include clinical, educational and support under the direction of your family physician. Home health care promotes independence and control of your life. We encourage family involvement.

Our telemonitors use state of the art technology to monitor vital signs and oxygen, keeping physicians informed of patients daily status.

Definition of home bound

The patient's condition must be such that leaving the home requires an extremely taxing effort.

In most instances a "homebound" patient's absence from home will occur for the purpose of receiving medical treatment.

Absences are infrequent, are of relatively short duration and do not indicate that the patient has the capacity to obtain health care provided outside rather than inside the home.

If the patient's condition, due to an illness of injury, restricts the patient's ability to leave home except with the aid of supportive device: cane, wheelchair, walker, maximum assistance of another person.

If patient's condition is such that leaving home is medically contradicted.

Any absence for religious services is deemed to be an absence of infrequent or short duration, and this does not negate homebound status.

Home Community Based Waiver Services

This program offers services through Medicaid to those patients who need assistance with activities of daily living. It includes light housekeeping, laundry, shopping and personal care.

This program is coordinated by a Registered Nurse who does case management and assists patients in finding the resources they need to improve their quality of life. This may include referrals to adult day care, meals on wheels or resources for obtaining medications.

All of these services require a physicians order and approval of the plan of care.

Medicare, Medicaid and most other commercial insurance companies cover home health care. Call our office for answers to specific questions or to find out how to arrange for services.
